You may use the following in your information letters and consent forms for future research that you plan to share with the Population Health Data Repository.

•Statement regarding Future Use of Data for Secondary Analysis Purposes

"Data resulting from this research may be retained for secondary analysis by additional researchers. Only data that has been deidentified (i.e. information such as names, addresses, phone numbers, etc. have been removed) will be retained. All participants will remain anonymous and their information will be kept confidential"

•Statement regarding Anonymity & Confidentiality

"When we report results of the <study>, no individuals will be identified. No one except staff from the <insert your research group here> will have access to the data. Any electronic data are located in locked offices. Paper copies of data will be stored in a locked facility at the <insert organization name here> and will be destroyed after 7 years. As well, the data will be electronically archived after completion of the study and maintained indefinitely after the research study has been completed. All information will be kept confidential."
